Employment Requirements

If you are extended an offer for employment, there are several employment requirements which must be completed prior to your first day of work. Employment is contingent upon successful completion of the Employee Health screening and completion of all of the requirements listed below.

Documentation of eligibility to work in the US

A detailed list will be provided once an offer of employment has been extended.

Background Checks

All new hires will be required to complete a comprehensive background check to include employment, education, Office of Inspector General, State Police, Sex Offender Registries, and references. Consents for such will be administered in the HR office on the date of the physical and HR paperwork appointments. In compliance with the Illinois Health Care Worker Background Check Act, certain positions will require a fingerprint background check. Details will be provided.

Official Transcripts

Please reference the job description that was provided to you in the interview, as this includes the educational requirements for the position.

Original copy of Licensure, Registration, and/or Certification

This is required for all positions which require a license, registration, or certification (i.e.  RN, Social Worker, Radiology Technologist, etc.)

Drug Screening

You will be required to provide a urine specimen at the time of your Employee Health visit.

Documentation of Measles/Mumps/Rubella (MMR) Vaccinations

If you are not able to locate your vaccination records, it is advised that you contact the high school or college from where you graduated or previous employer, as they will likely have this documentation. Blood titer results are acceptable forms of proof as well as from your own physician.

Documentation of any recent TB tests

Please note that TB testing will be required as part of the physical. If you have a record of a recently completed test, please bring it with you on the date of your physical.
